Madinah should be prefixed with al (alif-laam), as it appears like that in hadeeths and Saudi channels. Madinah is an indefinite noun whilst adding 'al' would restrict the word to the City Of The Prophet (SAWS). Also its better to say Salat al Fajr (Mudaaf+Mudaaf Ilaih), rather than Assalaat alfajr
